Note: This list is alphabetical, with the school districts/§4201 schools at the top of the list and the counties at the bottom. Only school districts/counties/§4201 schools on this list will be allowed to claim for speech services and special transportation services.
Annual Recertification for School Districts/Counties/§4201 Schools
Medicaid providers that submit electronic claims to New York Medicaid are required to submit a signed and notarized certification statement on a yearly basis. This certification process links the provider's assigned Medicaid identification number (NPI) to the Electronic Transmitter Identification Number (ETIN) under which electronic claims are submitted.
The Computer Sciences Corporation (CSC) sends out the annual recertification form to providers submitting electronic Medicaid claims. The recertification form needs to be completed by each school district/county/§4201 school that submits electronic claims to Medicaid. CSC sends out the recertification reminder 45 days prior to the school district/county/§4201 school decertification date, and then again 30 days prior to decertificaton date. Certification is good for one year, so each school district/county/§4201 school will receive their reminder(s) based on when they were certified last year. Failure to recertify annually will prevent your school district/county/§4201 school from billing Medicaid in the future.
