Items to Remember When Completing Forms

Budget Summary and Certification:

  • Complete and return this form as quickly as possible, and no later than the due date. 
  • Chief Administrator’s signature:
    • Original signature required.
    • Blue ink is preferred. 
    • Do not use markers.
    • CFO should not certify as there should be a segregation of duties.
    • Electronic, stamped, or photocopied signatures are not acceptable; any forms received with an invalid signature must be resubmitted with an original signature.  This causes delays in disbursement of funds.
