The 20% Verified Chargeback Listing(s) – Section 4408/4201 for July & August 2021, 2022, and 2023 reports reflect STAC Online (EFRT) System data, as of 5/5/2026. Each report is a listing of students where your district is identified as the District of Current Location for NYS Office for People with Developmental Disabilities (OPWDD) Chapter 47, 66, or 721 and Commissioner’s Appointment Section 4201 students. The “District of Current Location” is where the parent(s) or legal guardian of those students resided as of the start of the summer program.
The chargeback(s) will be accomplished by reducing your district’s state reimbursement to Summer 2025 Section 4408 Approved Payment Report (APR) #02 for Education.

Note: The “District of Current Location” listed on a student’s summer record may change from one year to the next. If a student’s parent(s) or legal guardian move, there may be a difference from the “District of Origin” legally and fiscally responsible for the 10-month OPWDD Chapter 47, 66, and 721 school year placements, which remains the same, regardless of where the parents reside after the student is placed by OPWDD.
