STAC ONLINE SYSTEM (EFRT) SCHOOL AGE APPROVALS

I. Overview

This screen is used by School Districts to data enter the information for a Request for Commissioner’s Approval of Services for Students with Disabilities (STAC-1) for 4408 Specially Designed Instruction and/or Related Services. Listed below are instructions and a table of data fields that need to be entered. Please view the DQPRG screen for a listing of approved 9015 programs and related service types for the selected education provider.

II. Accessing the DSSRS Screen

  1. Load EFRT in your web browser: https://efrt.nysed.gov/efrt/
  2. Log onto the STAC Online System. All entries must be uppercase.

i. Enter your assigned User Code.

ii. Tab to the Password field and enter your assigned password.

iii. Press Enter.

  1. From your home menu, select “DMNUS” ADD/AMEND REIMBURSEMENT APPROVALS.
  2. From the DMNUS menu, select “DSSRS” SPEC INSTR/REL SERV (2MTH) SUM 2014-FORW

III. 9015 Program Key

Code Type Required Work
9015-A RELATED SVCS ONLY Complete the Related Services section.
Select blank line from the Specially Designed Instruction dropdown.
9015-B SPECIAL INSTRC ONLY Complete the Specially Designed Instruction section.
Select blank line from the Related Services dropdown.
9015-C SPEC INSTRC W/REL SVCS Complete both the Specially Designed Instruction and the Related Services sections.
9015-D HOME/HOSP INSTRUCTION Complete either the Specially Designed Instruction section or the Related Services Section or both.
Select blank line from the dropdown of the section NOT being completed.

IV. Entering Reimbursement Approval

  1. Enter the student’s STAC ID in the STAC ID field.
  2. Enter the first three letters (with all uppercase) of the student’s last name in the Name field.
  3. Select the enrollment year from the School Year dropdown list.
  4. Click the Inquire button.

The student’s full name will be displayed for you to verify, and “Add” will be inserted in the Mode field at the top-right corner of the screen.

  1. Fill out the fields listed below:
Data Field Description
Disability Select the student’s disability from the dropdown list.
CSE District Pre-filled with your district’s 12-digit SED code.
District of Residence Select the district in which the student’s parent or legal guardian resides.
Agency To Be Paid Select the school district to be reimbursed by the State Education Department (must match either the CSE or residence district).
County of Residence Select the county in which the student’s parent or legal guardian resides.
Provider Select the approved provider from the list. Submit a Request for Addition to Drop Down Menu Form PDF form if the provider is not listed.
Program The approved 9015 programs for the selected provider will be populated in the SDI and RS sections of the screen.
Specially Designed Instruction
(Required for Programs 9015-B and 9015-C and, if appropriate, program 9015-D.)
  • 9015-B - Complete SDI section only and select blank line from the Related Services dropdown.
  • 9015-C - Complete both the SDI and Related Services sections of the screen.
  • 9015-D - Per the student’s IEP, complete the SDI, the RS or both sections of the screen. If only one section is being completed, select the blank line from the dropdown under the section of the screen that you are not completing.
Start Date Enter the date (mm/dd/yy) that the student first attended the program for the selected enrollment period. The student cannot start prior to the program begin date indicated in the program description.
End Date Enter the date (mm/dd/yy) that the student last attended the program for the selected enrollment period. The student’s enrollment cannot go beyond program end date indicated in the program description.
Group Size Select the appropriate value (between 1 and 5) from the Group Size dropdown list.
Number of 1/2 Hour Units Enter total amount of specially designed instruction the student received, in half-hour units.
Prorated Number of 1/2 Hour Units Display field only. System will prorate the number of half-hour units of specially designed instruction by the group size and display the calculated number of prorated ½ hr units once the record has been added.
Related Services
(Required for programs 9015-A and 9015-C and, if appropriate, program 9015-D)
  • 9015-A - Complete Related Services section only and select blank line from the Specially Designed Instruction dropdown.
  • 9015-C - Complete both the SDI and Related Services sections of the screen.
  • 9015-D - Per the student’s IEP, complete the SDI, the RS or both sections of the screen. If only one section is being completed, select the blank line from the dropdown under the section of the screen that you are not completing.
Start Date, End Date, Group Size, Number of 1/2 Hour Units Follow the same procedure outlined above for these fields from the Specially Designed Instruction section above.
Prorated Number of 1/2 Hour Units Display field only. System will prorate the number of half-hour units of related services by the group size and display the calculated number of prorated ½ hr units once the record has been added.
Transportation Enter Transportation cost (if appropriate).
  1. Click the Add button to submit.
  2. Confirm that the Total Num of Pro-Rated (SDI and RS) 1/2 HR Units and Cost have calculated correctly.

V. Amending Reimbursement Approval

  1. From your home menu, select the top choice on the list:
    DMQRY” STAC SCHOOLAGE&PRESCHOOL INQUIRY SCREENS
  2. From the DMQRY menu, select “DQCLD” VIEW/AMEND CHILD SERVICE APPROVALS.
  3. Enter the student’s STAC ID in the STAC ID field.
  4. Click the Get Profile button.
  5. Select the service approval you want to amend by clicking on the "S" to the left of the approval.

The service reimbursement approval will be displayed.

  1. Delete all of the text in the Go to field at the top-right corner of the screen.
  2. Hit Enter on your keyboard. “Change” will replace “Inquiry” in the Mode field just under the Go to field at the top-right corner of the screen.
  3. The fields that can be amended are listed below:
Data Field Description
Disability Select the student’s disability from the dropdown list.
Start Date Enter the amended start date.
End Date Enter the amended end date.
Group Size Select the amended group size from the dropdown.
Number of 1/2 Hour Units Enter the amended number of half hour units for Specially Designed Instruction and/or Related Services.
Transportation Enter the amended transportation cost.
(Transportation cost may also be amended on the DVST2 verification screen.)

For Related Services, ensure that there is a capital “C” in the Act box for any amended rows. Type a capital “A” in the Act box when adding additional Related Services to an existing DSSRS approval.

  1. Click the Change button to submit.
