Return of Approved Records and
Error Code Translation for Disapproved Records from
Submission of Electronic Approvals to STAC System
Files submitted electronically to SED for the purpose of entering child service data onto the STAC system will go through a series of edits before being approved. Those records that are approved will be returned to you with "00" in the SRV-TR-REC-STATUS field. If you sent us a STAC-ID then that STAC-ID will be returned to you in the SRV-TR-STAC-ID field. If you did not send in a STAC-ID then there will be a STAC-ID in SRV-TR-STAC-ID field that was assigned from already existing Child record or was from a newly created Child record. Approved records will also contain values in the record number field SRV-TR-REC-NO and the amendment number field SRV-TR-AMD-NO. These fields along with STAC-ID specifically identify a service approval record.
Those records that are rejected will have an error code from "01" through "ZZ" in the SRV-TR-REC-STATUS field. Below is a table explaining what these error codes mean.
| Error Code | Description |
| 01 | STAC-ID Not Found. The SRV-TR-STAC-ID you sent us was not on the system. If you do not know the child STAC-ID or this is a first time service, then this field should be=spaces. |
| 02 | Invalid Year. SRV-TR-YEAR should consist of two consecutive two digit years. For example, the 1996-97 school year should be represented as 9697. |
| 03 | Invalid Name. First name and last name must be entered (Middle initial can be spaces). The names may include only letters. There may be no spaces imbedded in either name. Do not include any surname suffixes such as III or Jr.) For example, Van Schaack must be sent as VanSchaack. Robert Jones III, Robert Jones 3 or Robert Jones Jr should be sent as Robert Jones. |
| 04 | Invalid DOB. Date of Birth must be sent in Century "19" until the year 2000 when it will be "20", Year, Month, Day order. The DOB must be a valid date that fits within the criteria of the service being processed. For example, a DOB of 19750630 for a 1996-97 4408 (summer) service would be invalid because the person would be 21 when summer school begins on 07/01/96. |
| 05 | Invalid Sex. Sex code SRV-TR-SEX must="M" or "F". |
| 06 | Invalid Racial-Ethnic Code. See table for valid values (see Specs for Submittal of Electronic STACs) . May not=spaces. |
| 07 | Invalid SSN. SRV-TR-SSN must=spaces or nine digits. |
| 08 | Invalid Outside ID. SRV-TR-OUTSIDE-ID must=spaces or be left justified and from five to nine characters. |
| 09 | Invalid Handicapping Condition Code (Disability). SRV-TR-HC-COND must=one of the values from the table (see Specs for Submittal of Electronic STACs). |
| 10 | Invalid Program Type. This is described in the Specs for Submittal of Electronic STACs. This is unique for each type of service. For example, this field (SRV-TR-PROG-TYPE) should="4408" for summer services. |
| 11 | Invalid Half-time/Full-time indicator. This field must be "Y" if it is a Half-time program and "N" if it is a Full-time program. |
| 12 | Invalid CSE Code. SRV-TR-CSE (Committee on Special Education District) must=a valid 12 digit code from the STAC Agency file that we made available to you. |
| 13 | Invalid CPLGR Code. SRV-TR-CPLGR (District of Parent or Legal Guardian) must=a valid 12 digit code from the STAC Agency file that we made available to you. |
| 14 | Invalid Agency To Be Paid. The SRV-TR-AGY-PD (Agency Paid) must=a valid 12 digit code from the STAC Agency file that we made available to you. |
| 15 | Invalid Education Program Codes. The SRV-TR-ED-PROV, SRV-TR-ED-PROG and SRV-TR-ED-STAC-ID should=a valid twelve digit Agency code, four digit program code and one character STAC-CD combination from the Program file we made available to you. This program must match the type of service approval you are requesting. For example, if SRV-TR-PROG-TYPE = "4408", then the program should be a 4408 (summer) program. |
| 16 | Invalid Education Dates. SRV-TR-ED-DT-BEG and SRV-TR-ED-DT-END must be valid dates in CC, YY, MM, DD order. Both dates must fit within the School year being applied for. Begin-date cannot be after End-date. |
| 17 | Invalid Education One-To-one Aide field. SRV-TR-ED-YN-1to1-AIDE=must="Y" if there is a one-to-one aide other wise this field should="N". |
| 18 | Invalid Maintenance Program Codes. The SRV-TR-MA-PROV, SRV-TR-MA-PROG and SRV-TR-MA-STAC-ID should=a valid twelve digit Agency code, four digit program code and one character STAC-CD combination from the Program file we made available to you. This program must match a valid maintenance program. |
| 19 | Invalid Maintenance Dates. SRV-TR-MA-DT-BEG and SRV-TR-MA-DT-END must be valid dates in CC, YY, MM, DD order. Both dates must fit within the School year being applied for. Begin-date cannot be after End-date. |
| 20 | Invalid Maintenance One-To-one Aide field. SRV-TR-MA-YN-1to1-AIDE=must="Y" if there is a one-to-one aide other wise this field should="N". |
| 21 | Incomplete Maintenance data. Submission of maintenance data is not required but if one field is entered then all pertinent maintenance fields described in the guidelines for the particular service being applied for must be entered. |
| 22 | Name/DOB does not match STAC Name/DOB. If you submitted a STAC-ID, SSN or Outside-ID upon which a key-match was made with a child record on the STAC system, and the name and/or DOB are significantly different, then the transaction will be rejected. The names must be at least a soundex match, and at least 2 out of 3 of the DOB=s year, month and day must match. |
| 23 | Overlapping Service Dates. There is already a service approval on the STAC system whose dates of service overlap the dates of service you requested. |
| 24 | Service Not Found. The key combination of SRV-TR-PROG-TYPE, SRV-TR-STAC-ID, SRV-TR-YEAR, SRV-TR-REC-NO you sent does not exist on the SED system. |
| 25 | Child record withdrawn. The Child record that matched either the SRV-TR-STAC-ID, SRV-TR-SSN or SRV-TR-OUTSIDE-ID you sent has been withdrawn from the STAC system. |
| 26 | Invalid Education Cost. The education cost field SRV-TR-ED-COST you entered must be a valid numeric value. This field is required for High-Cost. |
| 27 | Invalid Transportation Cost. The transportation cost field SRV-TR-ED-COST you entered must be a valid numeric value under 125,000. |
| 28 | STAC-ID withdrawn. While the STAC-ID you sent matched a STAC-ID on the STAC system, the STAC-ID had been withdrawn and is no longer valid. |
| 29 | Invalid Maintenance Provider. SRV-TR-MA-PROV must=a valid 12 digit code from the STAC Agency file that we made available to you. |
| 30 | CSE must be a Public School District. SRV-TR-CSE must be a valid Public School district on the STAC Agency File that we made available to you. |
| 31 | CSE Cannot be a Special Act School. SRV-TR-CSE cannot be a Special Act School on the STAC Agency File that we made available to you. |
| 32 | County Of Residence Must Be Valid. County of Residence must be a valid 2 digit county code between 01 and 68, inclusive. |
| 33 | CPLGR must be a valid Public School District. SRV-TR-CPLGR must be a valid Public School district on the STAC Agency File that we made available to you. |
| 34 | CPLGR can NOT be a Special Act School. SRV-TR-CPLGR must NOT be a Special Act School on the STAC Agency File that we made available to you. |
| 35 | Agency-Paid must=CSE or CPLGR. For certain services the SRV-TR-AGY-PD must=SRV-TR-CSE or SRV-TR-CPLGR. |
| 36 | There are already two education halftime records. You cannot add a halftime record if there are already 2 on the system for this school-year. |
| 37 | No Maintenance Allowed, Day Placements Only. You have attempted to add maintenance to a service where the Education Provider indicates that there are only day placements. |
| 38 | 4410 Invalid County Paid. The County-Paid field for 4410 Service must be a valid 2 digit Code between 01 and 68. |
| 39 | 4410 Invalid Hours Per Day. For 4410 Center Based Education, the SRV-TR-HRS-PER-DAY must be a valid numeric value=to or < the Hrs-Per-Day on the program record. |
| 40 | 4410 Invalid Days Per Week. For 4410 Center Based Education, the SRV-TR-DAYS-PER-WK must be a valid numeric value=to or < the Days-Per-Wk on the program record. |
| 41 | Invalid Education Group Number. For SEIT Education, SRV-TR-ED-NUM-GROUP must be a valid number between 1 and 10. |
| 42 | Invalid Education Sessions. For SEIT Education, SRV-TR-ED-SESSIONS must be a valid number=to or < the education sessions on the program record. |
| 43 | 4410 Invalid Foster Care County. The Foster-Care-County field, if entered for 4410 Services, must be a valid 2 digit Code between 01 and 68. |
| 44 | Invalid Related Service Program Codes. The SRV-TR-RS-PROV, SRV-TR-RS-PROG and SRV-TR-RS-STAC-ID should=a valid twelve digit Agency code, four digit program code and one character STAC-CD combination from the Program file we made available to you. This program must=that of a valid Related Service Program. |
| 45 | Invalid Related Service Type. SRV-TR-RS-TYP-SERV must=a valid Related Service Type such as OT, PT, ST etc.. |
| 46 | Invalid Related Service Dates. SRV-TR-RS-DT-BEG and SRV-TR-RS-DT-END must be valid dates in CC, YY, MM, DD order. Both dates must fit within the School year being applied for. Begin-date cannot be after End-date. |

| 54 | Program Record Not Found. The combination of (SRV-TR-ED-PROV, SRV-TR-ED-PROG and SRV-TR-ED-STAC-CD) or combination of (SRV-TR-MA-PROV, SRV-TR-MA-PROG and SRV-TR-MA-STAC-CD) did not match any program record on the STAC system. |
| 55 | Invalid Program Category. The program category on the STAC system is not consistent with the program requested. |
| 56 | Invalid STAC Code: The program STAC Code on the STAC system is not consistent with the program requested. |
| 57 | Program Not Approved. The program that you have requested has not been approved. |
| 58 | Program is Disapproved. The program that you requested has been disapproved. |
| 59 | Education Begin Date > Education End Date. SRV-TR-ED-DT-BEG > SRV-TR-ED-DT-END. |
| 60 | Education Begin Date < Program Begin Date. SRV-TR-ED-DT-BEG is less than the Program Begin Date on the STAC system. |
| 61 | Education End Date > Program End Date. SRV-TR-ED-DT-END is greater than the Program End Date on the STAC system. |
